Tidy up of index-user, my journals and account pages.

• journals can now be listed with <username>/journals from main menu.
• Removed list of journals from index dashboard.
• /index-user (dashboard) has slightly different display for local and volunteer,
• more helpful guidance for registering a new user.
• more guidance if local has not created an areas or volunteer is not watching any areas
• Still need a wizard for guiding through create a new area process.
• There are no changes in this release to the earth-engine API.
